It was the first Billy Joel album that I ever bought, and the cover kind of creeped me out. Forty some years later I see it more as introspective.
There's a stranger within each of us that stays buried deep and undisclosed. The song goes on to say that we don't know why it's there, or why it is that we keep a part of ourselves hidden, and most importantly, that it's not necessarily a bad thing.
